2 Primary Criteria Needed.

Very Severe, Severe, Moderate, Mild – Care Term Authorization.

Based on patient risk stratification patient will be seen 1 to 2 times a month by respiratory coach.

Regular evaluation by RT will determine patients breathing status - Improvement, Stability, and Decline.

Patients will have reported outcomes on a monthly basis showing breathing status and readmissions data.
